The body is a map. The key to long-term wellness is following that map and listening to it. Together, we'll move through your physical history, movement analysis, palpation, and several special tests...all designed to craft a unique care plan to resolve your specific issues.

Implement Your Plan

Each session we'll check-in on your body to ensure we're making real progress bases on your specific needs and goals and to ensure we conduct all our exercises and therapies with efficient and safe form.

A Take Home Roadmap

Once the plan of care is established and improvements have begun, it is important as a patient that you have a "roadmap" to take home with you. With videos from the session with cues that helped with a breakthrough and/or notes tailored to your progress, your plan will be easy to access using your phone or computer at home! 

ALWAYS Listen to the Patient

By this point we'll be able to assess to what extent the plan of care has improved your life based on feedback from listening to YOU. During our time together, there may be deviations to the plan because life happens! But we will always discuss why it's okay to deviate as long as your current holistic health is the priority. Once you're satisfied with your quality of life, I'll help you make sure the simple changes to your existing habits fit into your routine and is sustainable, so you can feel better without needing me there indefinitely.

I spent a few days a week with him for a few months, and he very quickly helped me to regain my range of motion and strength in my arm. He is very knowledgeable and takes an active interest in your recovery.   • I had a really great experience working with Jacob. I was having a lot of shoulder pain, to the point where I had to pick up my arm to move it at night. My girlfriend suggested physical therapy, but I didn’t really believe in it before. I have a very physical job, and I really started to see the effects of lack of function in my arm. I eventually gave into my girlfriend’s suggestions, and I’m so glad that I did. My pain and mobility improved quickly when I started working with Jacob, and now I don’t have any problems with my shoulder. I can lift weights and work without any issues.   •  initially met Jacob because I was accompanying my boyfriend on his physical therapy sessions. I was so impressed by his dedication to his work, his expertise, and his ability to tailor a specific program to the specific needs of his patient. I had been working with a different physical therapist before for some ankle issues, but I was discharged a bit early and still experienced a lot of pain and some limited mobility. I decided to schedule a consultation with Jacob and am very glad that I did. In a shorter period of time, he has really helped me with ankle issues that I have been dealing with for a while. What can you expect from the initial consultation?

To put simply, you can expect the formation of a roadmap through specific checks related to your pain and/or dysfunction formed by your story/history, movement analysis, palpation, and special tests. This road map will serve to both build the first steps of treatment moving forward and compare progress to in the future. 

What should you wear?

I recommend athletic, loose fitting clothing that allow you to feel comfortable doing movement and allow me to get to the areas I need to get to in order to help out.

How Long are the sessions?

The initial evaluation must be 60 minutes in order for me to form your plan of care with specificity and individualized care. From there, I offer either an avenue of follow ups with 60 minute treatments or an avenue that will be 30 minutes depending on a multitude of factors. As always, I am committed to working with your needs to find whichever avenue is both effective and accessible.

Do I accept health insurance? 

I operate as a cash-based (out of network) rather than accepting insurance directly. This approach allows me to offer a higher standard with 1-on-1 sessions for longer periods of time, individualized care, and a direct patient-therapist relationship.

Unlike other clinics who take insurance that juggle multiple patients within a one hour time period, I only see one patient at a time. While this may mean a patient pays out of pocket, it often leads to more efficient and effective outcomes, which could save money in the long run.

For those with insurance, I could offer a superbill for potential reimbursement if the insurane company reimburses out of network specialists. I am committed to working with you to find a care and payment strategy that makes your therapy accessible. 

Do I accept credit cards, debit cards, HSA cards, cash, or checks?

Yes to all of the above.

What is my cancellation policy?

The cancellation policy within 24 hours is full price due to the fact this is 1-on-1 treatment with a reserved time for the room and the therapist specifically for you. I understand extenuating circumstances may happen, but I kindly ask for your cooperation to avoid late charges and enable me to offer your time slot to another patient who may need it.
